‘I Wasn’t Able to Even Unseal My Eyes. That’s How Swollen I Was’

Onemanda Preisinger feels anxious about her daughter’s upcoming 13th birthday party. Not for the usual reasons associated with a house full of clamorous preteen kids, but since it’s the first time she will reveal her new face to acquaintances and relatives. “Clearly I’m going to inform everyone as they arrive, ‘Just so you know, this is not how I look,’” states the thirty-year-old property agent from Southern Florida.

Her appearance is, admittedly, a somewhat startling – her face swollen and unnaturally tightened, as though held together by industrial-grade tape. Her altered – and she’s keen to stress, temporary – look is the result of six cosmetic procedures, such as an endoscopic mid-facelift, performed by a surgeon in Turkey’s Istanbul, last month. “My spouse teared up when he saw me for the initial time because I couldn’t even unseal my eyes. That indicates puffy I was,” she explains to me via video call from her home. “I had to tell him: ‘Babe, I’m fine, I’m not hurting. I just look like someone jumped me.’”

Growing Trend of Early Facelifts

According to cosmetic specialists, Preisinger is among a rising count of people electing to have a facelift in their twenties and thirties – significantly before a decade prior to most doctors’ usual candidate age of 40 to 60. (Although many specialists are eager to highlight the industry edict of: “We address genetics, not years.”) “I have 28-year-olds asking for facial lifts,” states London-based cosmetic surgeon Georgios Orfaniotis, a former NHS consultant who specialises in the facial area. “It’s a very significant procedure, and I’m a bit concerned when I see people choosing it as a personal preference.”

Understanding the Rhytidectomy Surgery

A rhytidectomy, alternatively called a facelift, lifts the ligaments in the face that start to sag as we grow older. “Human faces are structured a bit like onion layers,” says based in Kent face specialist Marc Pacifico. “Dermis and adipose on the surface, then a layer of supportive tissue known as the SMAS. Think of the facial framework as the soft tissue skeleton of the face. And that’s what ages us; that is what becomes lax and pulls the dermis down with it.”

Previously reserved of moneyed older people, overuse of injectables – when excessive use of dermal fillers expands the face and leads to laxity in the dermis – combined with the widespread use of slimming medications, cut-price surgical travel, and the development of new, famous-figure-promoted operative methods are driving a different type of patient towards this major operation. Statistics indicate an 8% increase in facial lifts over the last year in the United Kingdom; while a survey found that the portion of youthful candidates is growing, with one-third of procedures now conducted for patients aged 35-55.

“People are now requesting to look like the optimal form of themselves and naturally the techniques are following,” says Orfaniotis. Currently, the trending procedure is the deep plane facelift, a method promoted by everyone from reality TV star Kris Jenner, to style creator Marc Jacobs.

Advanced Lifting Method

While a conventional rhytidectomy entails lifting the superficial layer, the comprehensive lift loosens the facial ligaments underneath, enabling doctors to reshape the face by moving the underlying layers, and preventing the tight, stretched look occasionally caused by older methods. “We avoid placing tension on the dermis because we’re going below, to the deeper tissue,” says prestigious London cosmetic doctor Rajiv Grover, who specialises in the deep plane technique. “It is the underlying layers that is repositioned. The dermis comes with it as a secondary element.” Lifting the whole facial structure as a single unit results in a authentic-appearing and longer-lasting outcome. It additionally implies that the procedure is no longer being used for the express purpose of anti-ageing – or rejuvenation, as the aesthetics industry describes it – but for “beautification” and facial restructuring, too. “We’re getting numerous requests from patients aged late twenties to mid-thirties for this purpose,” notes Grover.

Individual Experience

Preisinger had not intended on undergoing a facial lift – at least not at the thirty years old. But years of using injectable gels in an attempt at “symmetry correction” meant that by the time she entered her late 20s, she was “damaged”. “I didn’t undergo this because I desired to appear youthful,” she says. “I did it because the injectable harmed my face. I wish I had never done it. If I had avoided the filler, I don’t believe I would be in this situation currently.”

Whether dermal fillers ever fully dissolve has long been a debated topic in the cosmetic field. “Research have indicated that not only do they rarely fully dissipate,” states Grover, “but they move and can block drainage pathways. One of the contributors to what we call ‘pillow face’ is also the fact that in a sense, the face gets somewhat fluid-filled because its capacity to drain bodily fluid has been diminished.” Although research into the area is early-stage, warnings on dermal fillers’ possible effects on the lymphatic system have been issued, and further research initiated. An esteemed facial plastic surgeon, speaking anonymously, says he would avoid using injectables in a client because of the risks they present. “A lot of doctors don’t want to talk about this, because the manufacturers who make filler can be quite forceful.” He’s been told accounts, he adds, of plastic surgeons being landed with court orders after raising their concerns.

Decision and Procedure

For Preisinger, after beginning injecting filler, she believed she had to keep adding more – particularly when it began moving to other parts of her face. She was just 28 when a nearby cosmetic doctor in the state suggested that a facial and neck surgery could be what she required to finally step off the injectable hamster wheel. After 24 months, she ended up choosing between a list of recommended hotels in Istanbul, sent to her by the clinic assistant at her surgeon’s office.

Initially, Preisinger had scheduled an upper and lower blepharoplasty – better known as blepharoplasty – but her surgeon advised her that a facelift was the right solution for the looseness she was experiencing in her face. She arranged a forehead lift, a mid-facelift and a neck lift. The day before her surgery, one day after she’d arrived alone in the country, she decided to add a lip lift. “Then he stated, ‘We will elevate the lower eyelid puffiness.’ Then he said, ‘I think if we remove some cheek fat, it will contour your face.’ Thus I ultimately adding two more procedures,” she says.
